As a Care Coordinator at MetaPhy Health, you will play a pivotal role in overseeing and optimizing our clinical operations to ensure exceptional patient outcomes and operational excellence.
Key
Responsibilities:
Provide patient education, patient support, and patient care via a telehealth platform as an extension of the physician's practice Serve as a liaison and coordinate information between providers, other clinical staff, and patients Coordinate the ongoing delivery and management of patient-centered comprehensive care plans Complete and initiate appropriate clinical reminders, consults, and alerts Utilize EHR to document all clinical observations, information, updates, and patient education Schedule, confirm and coordinate virtual patient encounters Demonstrate an ability to listen and communicate with others in a professional and caring manner including sensitivity to individuals Demonstrate an ability to set priorities for tasks to work effectively despite interruptions and under minimal supervision
Qualifications:
Active Certified Medical Assistant or Registered Medical Assistant Certifications 1-2 years of experience in a healthcare setting (preferred) Legally Authorized to Work in the United States Working at MetaPhy Health:
